Post-nights shift

My last week of nights as medical SHO.Uh,uh..

Its been a good week. I really enjoy working with the team this week. The Spr is fantastic, always try to give me opportunity to do things. My other colleague is 3rd year SHO whom I work with in my house officer year. I really appreaciate it when my Spr said,"You are really good this week; all your patients seemed to be doing ok. And I don't really need to review them".

I think its been a good week as well. The cases are ok, nothing that is really out of my depth, although I probably have a growing concerns with the referral from A+E. Things that should have been pick up by them. My colleague gave them a good telling off.

Attempt to put a drum catheter for amiodarone infusion. And the size of the needle is massive - its even bigger that orange venflon (its like 28 or 30G!! probably exagerrating here. What is the size of the orange venflon?? sth like 25G is it?)! Unfortunately that is the only thing the ward has. End up with the reg putting a femoral line, as my drum catheter could not get pass into the brachial vein. I think its because the catheter is so big, it get stuck in one of the junctions above the antecubital fossa. He wanted me to put the femoral line, but my bleep went off for a crash call in HDU.

Its been a good week of oncall.

munchausen's

Interesting case - highly2x suspicious of munchausen's syndrome & a bed-hopper. Ie this person is usually someone who basically go around different hospitals, create story about symptoms to try get some attention or analgesia in the form of morphine. Ie: wasting NHS money. The chap we saw has a "midline" sternotomy scar supposedly from open heart surgery. When you feel his chest, it was smooth, no clips present at all (confirmed by CXR); feel more like a self-created scar. Bizarre.

selamat hari raya

Selamat Hari Raya Aidilfitri! Maaf Zahir Batin from both of us, huda and nik.

Esok start keja 12hour shift malam for 7 nights. La..la...la..dam di dam.
Admission rate to MAU has doubled and tripled recently.dam di dam.

I have got 2 audits, should start analysing datas, but can't be bothered now. lalala.

My raya mood dah lama masuk longkang.

Looking forward to my night shifts. Ha!

P/S: passing speculum seemed too mundane now.

Pride and Prejudice

I am not someone who is really into classics. The only classic I properly read and in fact enjoy is Jane Eyre by Charlotte Bronte (I even cried for heaven sake).

Just watch 'Pride and Prejudice' starred by Keira Knightley. And oh! Its so glorious; It blown me away. Now I can understand the enchantment with charming Mr Darcy(me gushing..). Bel, Dalie (if you guys read)I truly recommended it to you!

Baru balik from service keta. Then tingkap keta belah driver takleh tutup. Bebulu betul dgn electric window ni.. Garage dah tutup now.

Mintak2x tak hujan la malam ni. Northwest weather asyik hujan je autumn ni,takkan nak pegi keja basah kuyup lak. Padahal bawak keta.

What a crap on call yesterday...

Was doing medical cover oncall yesterday, soooo exhausting. I had 2 crash calls - one from psychiatry ward and orthopaedic ward. Thank God it wasn't sth that is really out of my depth; its just seizures & an apnoe & off colour episode (nobody needing intubation etc, phew!). Both wards are like "10 minutes usual walk". Jauh gila. Kena lari pecut, by the time we arrive I thought I am the one who gonna "arrest".

The second crash call came sth like 25 minutes before I was about to finish my work (and at the time I was waiting for a patient to be transferred to HDU from the ward for Type 2 respiratory failure; and he was like can go 'arrested' at any time so I have to basically escorted him during his transfer); and I was thinking - can it get any worse than this? There is like two pancytopenia needing transfusion as well.

I was oncall on Friday and its bloody depressing as well. An ischaemic bowel with ver bad chest, bad kidney, bad heart hence inoperable. Also seen someone with GCS 4/15 and respiratory failure and in the process of dying. And I feel so bad because they were all have reach the ceiling of treatment. Nil else I could do.

This week I have been facing the ugly side of medicine. One thing I did learn - identifying "dying process".

Imagine becoming medical registrar and have to make this kind of decision....

Mencarik restaurants....

Just come back from a night out...

How disappointing the Italian food. The pasta is way too soggy. The till doesn't work, so have to go out to find a cash machine. The waiters don't know where the nearest cash machine. Grr...

I just find it a little to hard to find a good restaurant in Manchester City Centre. A restaurant that actually make me want to go there again. (ok, lets exclude Nandos and Wagamama. That's franchise). I am tired with Indian food. Its good, but I want something else. New Samsi (Japanese) ok la. Fusion food is big in Manchester; it doesn't work very well with me though. Even Malaysian food is 'fusion' (Malay + Thai + Vietnamese + Indonesian). Why can't we have a proper proper Malay food like Naha and Mawar? We went to have soto in this Malaysian restaurant and boy, what a disappointment. Banyak gila bunga cengkih and there is santan in it???? And what with the price? Pricey.

It kinda make me missed London. You can just jalan2x and find a 'surprise' restaurant in a little corner or in an alley with a good food. Better than going through restaurant guide. Better than going to big shopping centre.

Masak sendiri lagi bagus.
